What it does

When you face a decision you cannot settle alone, this skill makes your agent interview you briefly about who you need input from and what you need back, then drafts a structured Markdown questionnaire aimed at that person's knowledge. The questionnaire is saved as a file in your current directory so you can send it for asynchronous or meeting-based completion. It is designed so the recipient can answer well without being in your head, with questions ordered by importance.

Great for

  • Managers or leads who need input from a subject-matter expert before deciding
  • Engineers gathering requirements or technical facts from a colleague or client
  • Anyone who wants a clear, reusable question document instead of a messy email thread

Skip it if

  • Simple questions you can answer yourself or look up, since the skill adds a questionnaire step that may not be needed
  • Sensitive or confidential topics where a written document could be forwarded or stored unexpectedly, because the file is saved locally in plain Markdown without any access controls

Try asking

  • “I can't decide our database migration plan alone. Help me build a questionnaire for our senior DBA.”
  • “Make a questionnaire for the client to fill in about their launch load expectations.”
  • “I need answers from our legal contact on data retention before we ship. Turn that into a questionnaire.”

    Download the skill

    The ZIP contains the 2 original files, the MIT licence and an attribution note.

    to-questionnaire.zip
  2. 2

    Add it to your agent

    Claude Code (all projects)

    mkdir -p ~/.claude/skills && unzip to-questionnaire.zip -d ~/.claude/skills/

    Claude Code / Agent SDK (this project only)

    mkdir -p .claude/skills && unzip to-questionnaire.zip -d .claude/skills/

    Claude apps (web/desktop): Settings → Capabilities → Skills → Upload skill, and choose the ZIP.

  3. 3

    Ask for the task

    No special command needed — the agent reads the skill's description and loads it when your request matches. Try one of the prompts above.

Get better results

  • Give the agent a concrete list of decisions you need resolved, since it writes questions against that list.
  • Name the recipient's role and expertise up front so the tone and depth of the questionnaire fit them.
  • Review the generated file before sending, since the agent may include questions you want to reword or drop.
